Print Patient Registration Form
To ensure maximum safety for our patients and staff, we continue to implement specific guidelines as advised by the Board of Optometry. We thank you in advance for your patience and understanding while we strive to maintain a safe and positive environment for all who enter. *UPDATED FEBRUARY 6th, 2023.
-
We get up close and personal during your eye exam, lens measurement and glasses adjustment... like 6 inches from your face at times. Patients and anyone accompanying the patient are ADVISED TO WEAR A FACE MASK. If they do not have one, we can provide one at your request. Anyone who has specific health concerns and requires specific accommodations, please call us and we'll be happy to accommodate you.
-
In order to have ample time to properly sanitize all equipment, frames and common touch points between patients, please schedule an appointment for your exam and glasses order. In-office pick-ups/adjustments/repairs can be done without an appointment. Please come at least 30 minutes before any closing time, as our closing times may change depending on staff availability.
-
Check-in for your appointment at our front desk, or call from your car. If you may be sick, have any cold/flu/Covid-19 symptoms, or a temperature above 100.4 degrees, we'll kindly ask you to reschedule.
-
We would love to meet your friends and family in the office. But because our office is small, we are asking patients to limit their number of companions (please leave pets at home).
-
Your eye exam appointment will be the only one scheduled for 30 minutes. We have a reduced schedule and never double book appointments. Therefore, we are implementing a no-show fee of $40 for any patient that does not show up for their scheduled eye exam. We kindly ask that you call/text us 48 hours in advance, to cancel or reschedule your appointment, so we can accommodate patients on our wait-list.

Our frames are sterilized within a fully enclosed UVC unit and/or cleaned with a hydrogen peroxide solution. Each patient is provided a tray to place all frames that they have touched or tried on, so they can be sterilized before returning to the display case. The dispensing area is disinfected frequently, including mirrors, chairs, and table tops.
